Care Management Assistant

McLaren Health Care - - Responsibilities: Assist with facilitating discharge planning to expedite a successful transition of care; Assist RN and SW Care Managers with the referral process by collecting and distributing informational brochures/pamphlets; Assist with coordination post-discharge placement and services by making referrals, confirming placement and service arrangements, processing, organizing and obtaining insurance verification/Prior Authorization; Assist with contacting community resources to coordinate discharge plans while maintaining professional communication; Perform concurrent and retrospective utilization management-related activities and reporting including IMM and MOON completion
