We serve people with Intellectual/Developmental Disabilities (I/DD), Serious Persistent Mental Illness (SPMI), and Serious Emotional Disturbance (SED).If you're passionate about supporting children and families, this is your opportunity to create lasting impact!-### **What You'll Do*** Provide case management and support services for children and youth up to age 21.* Collaborate with families, schools, and community partners to ensure comprehensive care.* Implement person-centered plans and monitor progress toward goals.* Maintain accurate documentation and comply with agency standards.-### **Qualifications*** **Education:** Bachelor's Degree (BA/BS).* Complete MDHHS Wraparound training.* **Have a criminal history screen, including state and local child protection agency registries.*** **Be supervised by an individual who meets the criteria as a Child Mental Health Professional (CHMP) who has completed MDHHS required training.**-### **Skills & Abilities*** Proficiency in Microsoft Office Suite and standard office technology.* Strong communication, organization, and time management skills.* Critical thinking and problem-solving abilities.* Attention to detail and accuracy in documentation.-### **Certifications Required*** Emergency Services (E.S.)
