Job Description:
Job Title: Licensed Practical Nurse (LPN / LVN)
Location: Utica, NY 13502
Shift Schedule: 8-Hour Night Shifts (10:30 PM - 6:30 AM) | Every other weekend required
Duration: 13 Weeks (Possible extension depending on business needs)
Guaranteed Hours: 37.5 hours/week
Pay Range: $34/hr - $44 per hour on a W2

Responsibilities:
Provide direct patient care under the supervision of a registered nurse or physician in a long-term care setting.
Administer medications and treatments as prescribed and document accurately in the EMR (PointClick Care).
Assist with daily living activities including bathing, dressing, feeding, and toileting.
Monitor patient conditions and report any changes to the supervising RN or physician.
Float to all units within the same building as needed to support care continuity.
Care for a 40-bed unit population including medical-surgical, orthopedic, dementia, and memory care patients.
Maintain accurate records of patient care, condition, progress, and concerns.
Ensure adherence to safety and infection control policies at all times.
Collaborate with interdisciplinary teams to deliver holistic and person-centered care.

Skills/Experience Needed:
Minimum 1+ year of recent experience as an LPN in long-term care - Required.
Experience with EMR systems, especially PointClick Care - Preferred.
Strong communication, observation, and clinical skills.
Ability to work independently, multitask, and float between units.
Comfortable with weekend and night shift rotations.

Education:
Graduate of an accredited Licensed Practical Nursing (LPN) program - Required.

Certifications:
LPN License - New York State - Required.
Basic Life Support (BLS - AHA) - Required.
