Bilingual Care Manager — Remote Patient Engagement Pro
Vigilance Health, Inc.
Vigilance Health, Inc. is hiring a Care Manager to lead patient outreach and engagement from a remote setting. You will build trust with a diverse patient panel, applying Motivational Interviewing principles and evidence-based practices to improve health outcomes.
You will coordinate care plans, prioritize high-risk patients, and document progress to support the care team. A bilingual clinician or health professional is preferred.
