Description

• Conduct evaluations in either inpatient or outpatient settings and develop tailored treatment plans for each patient
• Implement physical therapy interventions focusing on mobility, strength, balance, and overall function
• Collaborate with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
• Provide education to patients and families regarding therapeutic goals, discharge planning, and necessary home exercises
• Carefully document evaluations, treatments, and patient progress per facility and regulatory guidelines
• Engage in care team rounds and interdisciplinary meetings to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) from an accredited program or equivalent degree

Certifications

• State licensure as a Physical Therapist
