Job Description

Job Description:

Case Manager/Discharge Planning – National Care Coordination Command Center
Plans, develops, assesses, and evaluates care provided to members
Collaborates with physicians, interdisciplinary healthcare team, and patient/family
Reviews all inpatient admissions within 24 hours; initiates discharge planning
Serves as liaison between inpatient and referral facilities/agencies
Provides comprehensive case management and advocacy for patients and families

Requirements:

Active Compact RN License
BLS Certification (AHA)
Previous Case Management or Discharge Planning experience required
