Position Overview Physical Therapist – Mapleton Community Home, Mapleton, MN. As a Physical Therapist, you will directly participate in physical therapy treatment interventions and services in accordance with state practice guidelines, primarily caring for adult and geriatric patients. Essential Key Job Responsibilities Provide screenings and evaluations to determine requirements for skilled physical therapy intervention relative to patient’s prior level of function, including gait, balance, mobility, range of motion, strength, pain, home assessments, discharge planning, and development of maintenance programs. Develop and implement treatment plans focused on functional and measurable outcomes, adhering to professional standards of practice. Monitor patient progress and adjust treatment plans as needed. Provide appropriate supervision for physical therapist assistants in compliance with licensure, regulatory and company requirements. Maintain accurate and up‑to‑date patient records. Collaborate with, coordinate, and participate in training programs for community staff, patients, families, and rehabilitation staff. Participate in continuous quality improvement activities. Requirements Successful completion of an accredited Physical Therapy program. Knowledge, Skills, and Abilities Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Strong problem‑solving and critical‑thinking skills. Compassionate and patient‑centered approach to care. License And Certification Current valid Physical Therapist license to practice in the appropriate state.
