Fort HealthCare is a patient-centered hospital and healthcare system serving individuals, businesses and families in Jefferson, Dane, and Walworth Counties. We are seeking an LPN to join our Inpatient Services team. Shift: Part time Day/PM shift including every third weekend, 16 hours per bi-weekly pay period. Responsibilities Provides direct nursing care to patients on the unit, reporting concerns, data, and abnormal findings to the RN. Performs specific tasks and treatments as directed by the RN. Documents care provided in the patient's electronic medical record. Collaborates with members of the care team to provide safe, quality patient care. Participates in patient treatment regimen, including administering medications (under the supervision of a physician and registered nurse). Interviews patient about current problems, medications, and allergies and reviews medical record for pertinent previous and current information. Communicates changes in patient behavior or health status to nurses, providers, and/or caregivers. Assists patient with eating, dressing, bathing, while obtaining data such as vital signs. Assists preparing patients for transfer or discharge. Requirements One year of healthcare experience preferred. Able to effectively communicate with care team, patient, and family members.
