Dependable Nursing Inc. is looking for a Part-Time Physical Therapist (PT) to provide skilled home health care to patients throughout East Phoenix and the East Valley .

What You'll Do
Complete in-home physical therapy evaluations
Develop individualized treatment plans based on each patient's needs and goals
Provide therapeutic exercise, gait training, balance training, mobility instruction, and other skilled interventions
Help patients improve strength, function, endurance, and safety
Educate patients and caregivers on home exercise programs and fall prevention
Monitor patient progress and adjust treatment plans as appropriate
Collaborate with nurses, physicians, therapists, and other members of the care team
Complete accurate and timely documentation
What You'll Bring
Degree from an accredited Physical Therapy program
Active Arizona Physical Therapist license
Current CPR/BLS certification
Home health experience preferred
Excellent communication and time-management skills
Ability to work independently in a home health environment
Reliable transportation and valid driver's license
