Registered Nurse - Home Health Case Manager
Hospitals on Incredible Health are actively hiring and accepting applications in the Portland, OR area for the following position: Registered Nurse - Home Health Case Manager. Nurses with experience in any of the following areas are strongly encouraged to apply: Catheter care and placement, Clinical pathway, Develop and implement personalized care plans, Educating patients and families, Navigator, Skin assessment and ulcer prevention, Utilization Review, or Wound and Stoma care.
Shift(s) available: day shift
Job types available: full time, part time, and per diem

Qualifications:
- ADN/ASN degree or higher from an accredited school of nursing
- Active and unencumbered Registered Nurse license in the state of Oregon
