Taconic at Hopewell is seeking a compassionate and skilled Part-Time Speech-Language Pathologist (SLP) to join our interdisciplinary rehabilitation team. This role offers schedule flexibility and the opportunity to make a meaningful impact helping residents regain communication, cognitive, and swallowing function.

The Speech-Language Pathologist provides evaluation, diagnosis, and treatment of speech, language, cognitive, and swallowing disorders for residents in a skilled nursing and rehabilitation setting. The SLP collaborates with the care team to improve safety, independence, and quality of life.

Key responsibilities include evaluating and treating residents with speech, language, cognitive, and swallowing disorders, developing and implementing individualized care plans, performing dysphagia assessments and recommending safe dietary modifications, collaborating with nursing, dietary, and rehabilitation teams, educating residents, families, and staff on communication and swallowing strategies, maintaining accurate documentation in accordance with regulatory standards, and supporting quality measures and patient outcome goals.

Qualifications include a degree in Speech-Language Pathology, a current SLP license, a Certificate of Clinical Competence (CCC-SLP) preferred, experience in skilled nursing or post-acute care preferred, and strong clinical, communication, and documentation skills.
