Primary Care Physician - PACE Program (Geriatrics)

Overview

A growing PACE (Program of All-Inclusive Care for the Elderly) organization is seeking a Primary Care Physician to provide comprehensive care to a geriatric population in a collaborative, interdisciplinary environment. This role focuses on delivering high-quality, patient-centered care to older adults while supporting their ability to remain living independently in their communities.

Physicians in this program work closely with an interdisciplinary care team to manage complex medical needs, coordinate specialty care, and develop individualized care plans for participants.

Schedule

Monday - Friday
8:00 AM - 5:30 PM

Rotating after-hours phone call coverage approximately one week at a time , shared among providers.

Compensation & Benefits

• Salary: $198,000 - $316,000
• 9% annual bonus
• 21 days PTO
• 6 days CME
• Malpractice insurance with no tail requirement
• DEA reimbursement if obtained upon hire
• W2 employment

Location

This role will split time between two clinical sites in the East Bay region:

• Concord, CA
• El Sobrante, CA

As the program continues to grow, physicians may be based primarily at one location.

Key Responsibilities

• Provide comprehensive primary care to older adult program participants.
• Conduct initial, semi-annual, annual, and periodic medical assessments .
• Develop and update individualized care plans in collaboration with the interdisciplinary team (IDT).
• Participate in care conferences, family meetings, and interdisciplinary discussions .
• Manage chronic conditions and acute medical issues for geriatric patients.
• Order and interpret diagnostic tests and prescribe medications as appropriate.
• Coordinate specialist referrals and follow-up care.
• Provide medical oversight for participants in nursing facilities when assigned.
• Collaborate with hospitalists to coordinate care for hospitalized participants and ensure smooth discharge planning.
• Document all clinical encounters in the electronic medical record.
• Participate in quality improvement initiatives and clinic meetings.

Qualifications

Required

• MD or DO with active California medical license
• Board eligible or board certified in Internal Medicine or Family Medicine
• 1+ year of experience working with geriatric patients
• Interest in working with frail or elderly populations
• DEA certificate or ability to obtain upon hire
• Basic Life Support (BLS) certification

Preferred

• Experience working in a PACE program
