Seek consultation as necessary. Monitor patient responses to the treatment plan and regularly communicate patient progress and possible goal revisions to the physical therapist. Document services provided and patient response/progress. Educate the patient and family/caregiver about patient deficits. Assist with patient discharge planning and provide information regarding appropriate selection and use of adaptive equipment and community support programs. What We’re Looking For Education : Associate’s degree required; graduate of an accredited program in a related discipline required. Licenses : Physical Therapy Assistant license in the state of Tennessee. Certifications : Basic Life Support (BLS).
