Job Details Job Location: Bristol Hospice Manchester, NH - Manchester, NH 03101 Position Type: FT Salary: $70,000.00 - $80,000.00 per year Shift: Day Responsibilities Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicate findings to the registered nurse and other members of the interdisciplinary group Provide an assessment in the patient's identified residence and assist when this is not safe; develop alternate plan as required Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse, and plan intervention based on evaluation findings Provide counseling to patient and family/caregivers as needed in relation to stress and identified coping difficulties; provide crisis intervention when necessary Assess and educate the interdisciplinary group on any special needs related to the culture of the patient and family, including communication, family role, space, and special traditions or taboos Maintain clinical records on all patients referred to social work Educate patients and families on, and assist with, preparation of advanced directives Provide information and referral services for patients and families/caregivers regarding practical and environmental needs Serve as liaison between patients/families/caregivers and community agencies Maintain collaborative relationships with organization personnel to support patient care Maintain and develop contacts with public and private agencies as resources for patient and personnel Participate in development of individualized plan of care, involve patient and family, attend regular interdisciplinary group meetings, assist team in recognizing psychosocial stresses on terminal symptoms Assist physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues Actively participate in quality assessment performance improvement teams and activities Assist family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities Perform other duties as delegated by the Clinical Supervisor Requirements Master’s degree in social work from an accredited university (eligible if hold undergraduate B.A., degree in Social Work; pay based on degree type) Minimum of two (2) years of documented supervised experience in health care, hospice experience preferred Understanding of hospice philosophy and issues of death/dying Certified Hospice and Palliative Social Worker desirable Flexible work hours and ability to travel throughout assigned Bristol Hospice service area Willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs Perks Tuition Reimbursement PTO and Paid Holidays Medical, Dental, Vision, Life Insurance, and more HSA & 401(k) available Mileage Reimbursement for applicable positions Advanced training programs Passionate company culture committed to the highest standard of care in the hospice industry EEOC Statement Bristol Hospice is an equal‑opportunity employer.
