Summary:

The Social Worker is a key member of the interdisciplinary team who provides and oversees the provision of psychosocial care for selected patients and families. Conducts bio-psychosocial assessments, provides intervention and treatment as indicated. Identifies high-risk psychosocial factors of patients/families that impact health status and helps staff understand the influence of those factors upon the course of medical care. Collaborates with and provides social work consultation to other disciplines within the healthcare setting and the community.

Responsibilities:

Supervises and/or trains new staff, students, and interns.Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives. Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.

Required to identify and intervene for suspected abuse including sensitivity to age specific concerns; physical/emotional/sexual abuse of children and adolescents; domestic abuse for adults; and caretaker/institutional abuse for senior adults. Must be able to manage crisis situations, demonstrating ability to quickly appraise the crisis situation and select appropriate intervention. Able to document direct service in patient's medical record after each significant encounter. Must demonstrate proficiency in use of the electronic medical record. Must be able to demonstrate good problem-solving skills, root-cause analysis, and principles of clinical performance improvement.
