Physical Therapist (PT)

We are seeking a licensed Physical Therapist (PT) to join our home health team. The PT will provide individualized rehabilitation programs to help patients regain mobility, reduce pain, and improve overall physical function in a home care setting.

Key Responsibilities

Perform comprehensive physical assessments of patients
Develop and implement individualized treatment plans
Provide therapeutic exercises and mobility training
Educate patients and families on recovery and prevention strategies
Monitor patient progress and adjust care plans accordingly
Maintain accurate and timely documentation
Collaborate with interdisciplinary healthcare teams

Requirements

Degree in Physical Therapy from an accredited program
Active Ohio PT license
Strong clinical and assessment skills
Home health or post-acute experience preferred
New graduates welcome

Work Details

Setting: Home Health
Patient Type: Geriatric / Post-acute
Schedule: Monday-Friday, 9:00 AM - 5:30 PM
Patient Load: 4-6 patients/day
Travel: 25-30 mile radius (mileage reimbursed)

Compensation & Benefits

Pay: $42-$58+ per hour
Mileage reimbursement: $0.40/mile
PTO, 401(k), pension, sick leave (eligibility applies)
