Pay rate range: $23-26/hour

The LPN - Population Health supports the delivery of proactive, patient-centered care by assisting with care coordination , chronic disease management, preventive health initiatives and quality improvement activities. Working as part of a multidisciplinary team, the LPN engages patients through outreach, education, and follow-up to improve health outcomes, close care gaps, and promote adherence to treatment plans. The LPN plays a key role in supporting value-based care initiatives, preventive screenings, care transitions and ongoing management of patients with chronic conditions. FLSA Status: Non - Exempt Created: September 2026 POSITION

Qualifications:

• Graduate of an accredited Practical Nursing Program
• Current, unrestricted LPN license in state of practice
• Current BLS certification
