Compensation: $250,000 to $325,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

As a Geriatric Rounding Physician, you'll care for patients where they live, follow them over time, and become an important part of their healthcare journey. You'll manage complex chronic conditions, coordinate care with interdisciplinary teams, and help patients avoid unnecessary hospitalizations through proactive medical management.

This is medicine built around continuity, relationships, and thoughtful clinical decision-making.

• Perform diagnostics, treatment planning, preventative care, and chronic disease management.

• Reduce avoidable hospitalizations and emergency room utilization through proactive intervention.

• Collaborate with Nurse Practitioners, Physician Assistants, nurses, therapists, and interdisciplinary teams.

• Communicate effectively with residents, families, caregivers, and facility staff.

• Maintain accurate documentation, medication management, and follow-up care.

• Active Minnesota Physician (MD or DO) license in good standing.

• Experience in geriatrics, internal medicine, family medicine, post-acute care, or related specialties preferred.

• Strong clinical judgment and comfort managing medically complex patient populations.

• Valid driver's license and reliable transportation.

• Commitment to high-quality, patient-centered senior care.
