Physical Therapist - Home Care - Full-Time

Schedule:  Full-Time , M-F 8am - 4:30pm (flexible hours).

Location: This position serves home care patients in Stillwater, MN, Washington County and St. Croix County (WI).

Pay rate:  Up to $78/hour Depending on Experience, mileage included!

*This is considered a temporary assignment that is an open ended vacancy, but candidates interested in direct hire/permanent or temp-to-hire will be considered as well.

What we ask of you:

• Assess physical, psychological, social and environmental needs of the patient and family.
• Integrate principles of development and disease process including co-morbidities in creating the plan of care.
• Maintain necessary records and use them appropriately for the care of patients.
• Provide patient/family education including patients self care and care provided by family.
• Serve as a case manager for assigned patients and coordinate with all providers involved in patient care including RN, SW, home health aide and other rehab disciplines.
• Assist patients and families in resolving concerns or complaints of care or service, and provide direction and supervision of care by home health aides.
• Assess needs and refer to Adult Protection Services regarding status of vulnerable adults as necessary.
• Provide skilled clinical care including evaluation and treatment of patients, and the development/implementation of care plans. 
• Anticipate the needs of the population served as well as the unique characteristics of the communities served.
• All other duties as assigned.

 

Required Skills/Qualifications:

• Must have a current license with the Minnesota Board of Physical Therapy and either have or be open to obtaining a WI license.
• Must have a Bachelors, Masters or Doctorate degree in Physical Therapy.
• At least  one year of previous experience as a PT in home care settings.
• Must demonstrate knowledge of age related growth and development principles to provide appropriate service regarding primary diagnosis and multiple co-morbidities affecting care.
• Proficiency with communication and data processing equipment, clinical apparatus related to home care, and electronic medical record system.
• Must have BLS certification through American Heart Association (AHA) or American Red Cross (ARC).
• Must have reliable transportation for client visits.
