LPN – Long Term Care

Part Time evenings (2x12s), every other weekend + holiday

Contract Length: 14 Weeks

Requirements:

• NY LPN Licensure
• BLS certification through AHA or RQI
• 1 year of experience as an LPN

Roles & Responsibilities:

• Collect and assess data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team.
• Contribute to the development and implementation of resident care plans that are culturally relevant and age-appropriate.
• Evaluate resident outcomes and recommend plan revisions, ensuring collaborative input from the resident, family, and healthcare team.
• Maintain competency in nursing practice through continuing education and mentoring of staff.
• Ensure safety and cost-effectiveness in planning and delivering care in collaboration with the RN.
• Provide leadership and clinical expertise within the scope of nursing practice.

Compliance / Notes: Must be able to begin full-day orientation.
