About the Role
As an RN Case Manager, you'll provide direct patient care, coordinate individualized care plans, and serve as a trusted presence for patients and families. You'll collaborate with an interdisciplinary team to ensure every patient is cared for - body, mind, and spirit. With a strong focus on clinical judgment, communication, and organizational skills, you'll be responsible for:
- Assessing patient and family needs and developing personalized care plans
- Delivering skilled, compassionate nursing care and symptom management
- Administering medications and coordinating care across the healthcare team
- Educating and supporting families to promote comfort and peace of mind
- Leading and guiding hospice aides to ensure consistent, compassionate care
- Maintaining accurate documentation and participating in ongoing education
What You'll Bring
- Current RN license in Virginia (multi-state or single-state)
- CPR certification and at least two years of nursing experience (hospice or home health preferred)
- Strong clinical judgment, communication, and organizational skills
- Reliable transportation, valid driver's license, and insurance
