Job Summary:
A Hospice RN (Registered Nurse) Case Manager position is available in Basking Ridge, New Jersey. This role involves performing patient assessments, developing care plans, ensuring appropriate utilization of services, and collaborating with an interdisciplinary team. Candidates must bring their RN license on the first day.

Job Details:

• Location: Basking Ridge, New Jersey
• Schedule: Days, Monday through Friday, 8:00 AM 4:00 PM, with every other weekend and compensatory days during the week. Schedule includes 37.5 hours per week.
• Required Qualifications: Valid New Jersey RN License; Associate's (ASN) degree or Diploma with RN License in New Jersey; 2-3 years of experience as RN in a home hospice environment; experience using an Electronic Medical Record (EMR); BLS certification; valid New Jersey drivers license; excellent critical thinking and communication skills.
• Preferred Qualifications: N/A
• Float: Float: Float rotation may be required to different counties according to need.
• Additional Information: Responsibilities include performing patient assessments, developing and revising care plans, ensuring medication safety, participating in performance improvement activities, and providing mentorship and orientation to new employees. The role requires adherence to hospice regulations and insurance reimbursement guidelines, maintaining a safe environment, and active participation in interdisciplinary team meetings.
