About the Position

We are seeking a compassionate and dedicated Travel Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) . The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide evidence-based therapy services to improve mobility, strength, balance, endurance, and overall functional independence.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans
• Provide therapeutic exercise, gait training, balance training, transfer training, manual therapy, and functional mobility interventions
• Assess patient progress and modify treatment plans to maximize rehabilitation outcomes
• Educate patients, families, and caregivers on home exercise programs, mobility techniques, fall prevention, and safe transfer strategies
• Complete accurate and timely documentation in accordance with facility policies, Medicare guidelines, and regulatory requirements
• Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other members of the interdisciplinary healthcare team
• Participate in interdisciplinary care planning and discharge planning
• Maintain a safe, clean, and organized treatment environment
• Ensure compliance with all federal, state, and facility regulations
• Deliver compassionate, patient-centered care focused on restoring function, promoting independence, and improving quality of life

Requirements

• Required: Graduate of an accredited Physical Therapy program
• Required: Active Physical Therapist (PT) license in the state of assignment or ability to obtain
• Preferred: Minimum of 1 year of Skilled Nursing Facility (SNF) or post-acute care experience
• Strong communication, documentation, organizational, and interpersonal skills
• Ability to adapt quickly to new clinical environments and work independently
• Commitment to providing high-quality, patient-centered care
