Minimum Requirements

• Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active Indiana State Physical Therapy License is Required to Start the Assignment
• BLS Certification from American Heart Association or American Red Cross may be required
• Outpatient clinic experience required
• Experience evaluating and developing treatment plans, including functional treatment programs and goal-oriented care
• Competence with outpatient documentation, goal writing, letters of medical necessity, and charge entry
• Ability to supervise and provide direction for discipline-specific therapy services and to participate in student training
• Preferred: Prior experience working in a Select Medical or similarly governed clinic environment
