In-Home Medical Day Care Provider - Weekdays (PRN - Flexible Hours)

ChildServe is seeking anIn-Home Medical Day CareProviderto provide personal care and supervision for an individual with special health care needs in their home. ScheduleVarious hours between 9am – 5pm M-F How You’ll Make an Impact Provide direct care and assist individuals with daily living and behavioral needs in their home Implement care plans and provide ongoing feedback regarding additional or changing care plan needs Perform duties related to the tracking and upkeep of participant’s personal belongings Responsible for timely and accurate communication regarding services with families, staff, and supervisors Transport individuals to outings in the community What You’ll Need: Must be at least 18 years old to apply Experience working with or knowledge of individuals with special health care needs preferred Provide your own transportation Must be willing/able to become an approved driver for ChildServe, if required Ability to electronically document services provided Ability to communicate effectively in reading, writing, and speaking the English language Ability to lift up to 35 lbs. be mobile within the assigned work area, and frequently bend, lift, reach, and stand Make a Difference Every Day at ChildServe ChildServepartners with families to help children with special healthcare needs live a greatlife.
