Join our dedicated team as Social Worker , where you'll lead with heart, advocate for residents, and help shape a caring, supportive community. Key Responsibilities Oversee and manage the Social Services Department, ensuring high-quality support to residents and families. Assess psychosocial needs and develop individualized care plans in collaboration with clinical teams. Lead discharge planning and transitions of care. Provide crisis intervention, counseling, and ongoing emotional support. Ensure compliance with all federal, state, and facility regulations. Supervise, train, and support social work staff. Serve as a liaison between residents, families, and interdisciplinary teams. Coordinate the facility’s outreach program or services to families of residents. Perform advocacy services on behalf of the residents to ensure that concrete needs are met, such asclothing, laundry, and the resident'spersonal needs allowance if one is maintained. Qualifications Bachelor's degree in Social Work (BSW) required; MSW preferred. Minimum 2–3 years of experience in long-term care or geriatric settings. Strong leadership, communication, and problem-solving skills. Knowledge of state and federal LTC regulations, including MDS and care planning. What We Offer Competitive salary and benefits package.
