Key Responsibilities\n- \n

Conduct thorough assessments of patients' psychosocial needs to develop individualized treatment and referral plans, including crisis intervention as necessary.

Assist patients in navigating the complexities of entitlements, insurance, and benefits applications that eliminate reliance on emergency assistance programs.

Facilitate access to prescription cost assistance initiatives provided by pharmaceutical companies and community resources to alleviate financial burdens on patients.

Serve as a patient advocate when liaising with community agencies to ensure appropriate support and resources are secured for patients' needs.

Maintain accurate and up-to-date patient records, preparing necessary documentation and reports to track progress and adherence to treatment plans.

Coordinate transportation assistance for patients to ensure they can attend appointments and access necessary services.

Engage in clinical and team meetings, contributing insights and expertise during case conferences to enhance overall patient care.

In your role, you will work with a diverse range of patients experiencing various mental health challenges, including anxiety, depression, trauma, and substance use disorders. Your therapeutic interventions will be tailored to support individuals from varying backgrounds, ensuring that your approach aligns with their unique cultural and personal experiences.

Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

Possession of one of the following active licenses: Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

A minimum of two (2) years of professional experience in mental health counseling, demonstrating strong competency in clinical skills and patient care.
