Clinical Case Manager — Housing & Recovery Advocate

DESC's SAGE program seeks a dedicated case manager to support adults with severe mental illness in securing housing, services, and stability. You will work within a multidisciplinary team to conduct psychosocial assessments, develop consumer-centered goal plans, and coordinate psychiatric and substance use treatments. Travel locally for outreach, provide crisis intervention and ongoing support, advocate with landlords and community providers, and maintain thorough clinical and administrative #J-18808-Ljbffr DESC
