Key Responsibilities

• Evaluate and treat patients with speech, language, voice, and swallowing disorders in a skilled nursing facility setting
• Develop and implement individualized treatment plans based on patient assessments and clinical goals
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Collaborate with interdisciplinary care teams to support comprehensive patient outcomes
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory requirements

Required Qualifications

• Active Speech Language Pathologist (SLP) license in the state of Colorado
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA preferred or eligibility required per facility standards
• Experience working in a skilled nursing facility or long-term care environment
• Strong clinical documentation and communication skills

Preferred Experience / Skills

• Prior experience with dysphagia evaluation and treatment, including modified barium swallow studies
• Familiarity with MDS/PPS documentation processes in a SNF setting
• Experience with geriatric patient populations
