Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on the specialty. License: Must have a current registered nurse license. Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase (HCHB) and OASIS within the last 6 months. Description: This position involves providing, coordinating, and directing home nursing care according to physician's orders. Care is delivered in accordance with agency policies and procedures. The registered nurse is responsible for applying the nursing process competently. The role requires direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses. Home visits will be required for patient care. Periodic office meetings may also be necessary. Reliable transportation, a current driver’s license, and required insurance are essential. The average number of patients seen per day will be between 5 and 10. The position aims to achieve a productivity expectation of 30 points per week. Each visit type and activity will hold a different point value, with approximately 6 to 8 points gained from patient visits. Types of cases will include Start of Care, Wound Care, and Follow-Up Visits. Clinicians will need to be willing to travel within the designated mileage radius. Documentation must be completed within 24 hours of patient appointments. Understanding the expectations around accepting all scheduled patients is crucial. The position may require participation in rotating on-call duties with other staff. Local candidates are welcome to apply. All candidates must be open to traveling up to a specified mileage from their primary location. No guaranteed hours are associated with this contract.
