Responsibilities:

Provide direct patient care including monitoring vital signs, administering medications, and wound care

Collaborate with RNs, physicians, and interdisciplinary care teams to develop and implement patient care plans

Document patient assessments, treatments, and progress accurately in medical records

Assist with patient admissions, discharges, and transfers in accordance with facility protocols

Educate patients and families on care instructions, medications, and health management

Respond promptly to patient needs and changes in condition, escalating concerns as appropriate

Maintain a safe, clean, and compliant care environment following all regulatory standards
