For more information, visit our website: Role Description This is a full‑time, on‑site PRN Physical Therapist role based in Lansdale, PA. The primary responsibilities include evaluating, diagnosing, and creating personalized care plans for patients to improve mobility, relieve pain, and prevent further physical challenges. The Physical Therapist will provide rehabilitation treatments tailored to individual patient needs, collaborate with interdisciplinary teams, and ensure optimal patient care both on‑site and through potential home care visits. This role requires attention to detail and empathy, with a commitment to improving patients' overall quality of life. Qualifications Proficiency in Physical Therapy practices and principles in a clinical or home‑based setting Knowledge of Medicine and Rehabilitation techniques to support patient recovery Strong experience in Patient Care, including developing and managing personalized care plans Ability to deliver Home Care services and adapt treatment plans to diverse living environments Licensure as a Physical Therapist in the state of Pennsylvania Strong communication and interpersonal skills Compassionate approach to working with individuals with diverse needs Ability to work collaboratively as part of a multidisciplinary team #J-18808-Ljbffr OneWell Health Care
