Transplant Social Worker - LGSW, LISW, LICSW

Join a collaborative transplant team as a social worker supporting patients and families through every stage of the transplant journey. This ambulatory role focuses on heart and lung programs, with opportunities to contribute across additional transplant services. You will provide clinical assessments, interventions, and care planning while partnering with an interdisciplinary team in a mission-driven environment. Key Responsibilities * Conduct psychosocial and diagnostic assessments to identify patient and family needs

• Develop and implement individualized treatment and transition plans
• Provide clinical interventions, including crisis intervention and

psychotherapy

• Deliver education and support to patients and their families
• This position is not eligible for visa sponsorship with the exception of the TN
