Overview FirstHealth Hospice & Palliative Care is a 5 STAR rated non-profit hospice offering compassionate end-of-life care support. As a Hospice RN Case Manager you will have flexibility in providing skilled complex care to the hospice patient and family in the community. The RN case manager will work collectively with members of the interdisciplinary team to facilitate and oversee a care plan and symptom management needs in the home environment. At least two years of experience in an acute care facility or home care agency. Must possess a valid NC driver's license.
