Key Responsibilities\n

• Conduct individualized therapy sessions for patients, tailoring interventions to meet unique needs, whether they be in group, family, or one-on-one settings.

• Implement crisis intervention strategies as needed, demonstrating poise and assurance during challenging interactions, while ensuring patient safety and emotional support.

• Complete thorough progress notes to track patient developments and therapeutic outcomes using established electronic health record (EHR) systems.

• Draft and execute individualized treatment plans based on comprehensive intake assessments for new clients, incorporating evidence-based methodologies.

• Collect, interpret, and use data effectively to enhance treatment approaches and support patient well-being—ensuring all interventions are success-oriented.

Patient Population / Client Focus\n

You will primarily serve adult patients navigating complex emotional challenges, including those dealing with terminal illness and end-of-life issues. Your role is pivotal in offering support, guidance, and therapeutic intervention that aligns with their emotional and psychological needs within the hospice care framework.

Qualifications\n

• A Master's degree in Social Work from an accredited institution, coupled with a current, active clinical social worker license (LCSW) in Pennsylvania.

• Demonstrated experience as an LCSW with at least 2 years of direct patient care, particularly in mental health or hospice settings.

• Solid understanding of crisis intervention techniques and the ability to apply them effectively in high-stress situations.

Compensation and Benefits\n
