Job Summary

We are seeking a skilled Residential Nurse Case Manager to support and enhance the overall health and well-being of individuals in our care. In this role, you will provide service coordination, advocate for individual needs, and offer education to ensure high-quality healthcare outcomes. You will work closely with individuals with developmental disabilities, brain injuries, and neurological conditions, contributing to consistent and effective care within the program.

Key Responsibilities

• Act as a central point of contact, advocate, and informational resource for routine and specialized healthcare needs, ensuring each individual has a personalized care plan.
• Promote a collaborative, team-based approach to delivering high-quality care by maintaining strong communication and coordination with multiple stakeholders.
• Evaluate and monitor the ongoing healthcare needs of individuals served.
• Partner with interdisciplinary teams to design and implement comprehensive care plans and service systems that are responsive and person-centered.

Qualifications

• Must be a Registered Nurse (RN) with an active and valid New Jersey license.
• Associate's or Bachelor's degree in Nursing required; additional certification in Case Management, Developmental Disabilities, Brain Injury, or as a Certified Rehabilitation Registered Nurse (CRRN) is a plus.
• At least two years of general nursing experience; candidates with three or more years of relevant experience are preferred.

Additional Requirements

• Monday to Friday 8am-4pm schedule.
