  • Conduct comprehensive evaluations and assessments to understand the specific needs of children and their families.

  • Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), tailoring interventions to promote optimal growth and development.

  • Monitor and document the outcomes of interventions as part of a committed team, ensuring compliance with all Federal and State regulations.
  • \n
  • Provide direct support and resources to families, equipping them with skills and knowledge to enhance their child's development.
  • \n
  • Establish and foster strong therapeutic relationships with children and families, creating a supportive learning environment.
  • \n
  • Utilize evidence-based practices to recommend strategies that align with the Natural Learning Environment Practices for children with developmental delays.
  • \n
  • Apply coaching models and natural environment teaching methods to increase family engagement and support effective learning.
  • \n
  • Maintain compliance with all safety and health standards to protect both our clients and team members.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker, you will play a vital role in serving children with disabilities or delays in our Hospice Center. The focus is primarily on children from birth to three years old, where you will collaborate with families to guide their development and help navigate challenges. Your work will significantly impact their physical, cognitive, social/emotional, and communication skills, ensuring the best possible outcomes for these young lives.

  • A Master's Degree or Doctorate in Social Work.

  • Current licensure as a Clinical Social Worker by the Department of Public Health.

  • A valid driver's license is required.

  • One year of specialized experience in early intervention with children is desirable.
