Job Description

Job Summary:
The Physical Therapy Assistant provides outpatient therapy services to patients, assisting in their rehabilitation and recovery. The role involves home health visits, documentation, and collaborating with healthcare teams to deliver quality patient care.

Responsibilities:

• Conduct home visits to assess and provide physical therapy interventions for patients.
• Document patient progress, treatment plans, and clinical notes accurately within required timeframes.
• Travel up to 75 miles between patient homes, ensuring timely and efficient service delivery.
• Collaborate with supervising therapists and healthcare providers to develop and update treatment plans.
• Participate in periodic office meetings and team discussions to coordinate care.
• Adhere to all compliance and documentation standards, including EMR system usage and travel regulations.

Qualifications:

• Hold an active Physical Therapy Assistant license in good standing.
• Demonstrate at least 1 year of recent adult home health or hospice experience.
• Proficient in HomeCare HomeBase EMR or similar documentation systems.
• Have reliable transportation, a valid driver's license, and current liability insurance.
• Capable of traveling up to 75 miles and accepting flexible patient schedules.
• Be willing to work in both home and care facility settings.
