Registered Nurse (Patient Aligned Care Team)

Summary The Cincinnati VA Medical Center is recruiting for a Patient Aligned Care Team (PACT) Registered Nurse (RN) at its Georgetown, OH Community Based Outpatient Clinic (CBOC). The PACT RN executes responsibilities that demonstrate leadership, experience, and creative approaches to managing complex primary care. The incumbent will provide sex‑specific direct Veteran care and care coordination to Veterans within the Primary Care Department. Qualifications English Language Proficiency – compliant with 38 U.S.C. 7403(f) basic written and spoken English requirement. Graduate of a professional nursing school approved by the appropriate accrediting agency (ACEN or CCNE). Or enrollment in a master’s level bridge program in nursing with coursework equivalent to a bachelor’s degree in nursing; may become licensed prior to graduation. Upon state license achievement, may be appointed on a temporary basis and later converted to a permanent appointment after graduation from the bridge program. Graduates from foreign nursing schools must possess current, full, active, and unrestricted registration and may be required to provide formal degree equivalency validation from a recognized evaluator such as International Consultants of Delaware. Graduate Nurse Technician (GNT) Exception – candidates lacking licensure may be appointed at the entry step of the grade for a 120‑day temporary appointment while pursuing licensure (may extend up to two years). Grade Determinations – education and dimension criteria for each grade and level are detailed on the online assessment: Preferred Experience – at least two years of outpatient nursing experience. Physical Requirements – duties involve direct patient care, walking, standing, sitting, moderate lifting, repeated bending, or kneeling. Duties The Cincinnati VA Medical Center (CVAMC) serves 15 counties in Southwest Ohio, Northern Kentucky, and Southeast Indiana from two divisions and six Community Based Outpatient Clinics, including Georgetown, OH. Responsible for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age‑specific components. Coordinating care focused on patient education, self‑management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Demonstrates leadership and creative approaches to managing complex client care. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access for patient panels. Provides health promotion, health education, and coaching on wellness, disease prevention, and chronic care management. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and guidelines. Provides constructive feedback for peer improvement and shares primary care knowledge to ensure safe, quality nursing care. Fosters a safe and supportive environment for professional development. Evaluates outpatient outcomes of evidence‑based decisions and practice changes for primary care individuals, groups, and populations. Documents PACT outcomes at the program or service level.
