The PT will deliver care in accordance with a physician’s plan, helping patients regain mobility and independence.**Responsibilities*** Evaluate patients’ functional status and develop care plans* Assess home environments and recommend modifications* Provide direct physical therapy treatments* Educate patients and caregivers on therapeutic techniques and equipment* Monitor progress and adjust treatment plans* Collaborate with physicians and interdisciplinary teams* Document clinical notes and progress reports* Participate in discharge planning* Supervise Physical Therapy Assistants per policy and regulations**Qualifications*** Active California Physical Therapy License* Minimum 2 years of physical therapy experience (home health preferred)* Strong communication and organizational skills* Current CPR certification* Valid driver’s license and insured, reliable vehicle**ABOUT THE COMPANY****Seaport Scripps Home Health**is a subsidiary of the **Pennant Group**network with 300+ locations throughout the country.
