Registered Nurse - Home Health

Job Description:

This RN position is for the field in Rapid City and will provide care for both home health and hospice.

Responsibilities:

Provide routine home health and hospice care; manage a caseload of 20-25 patients with weekly visits.
Coordinate with an LPN to ensure comprehensive care management.
Utilize Epic Remote for charting and documentation.
Participate in weekend rotations and on-call scheduling.

Requirements:

Minimum 1.5 years of experience in home health and hospice.
BLS certification and a valid driver's license.
Completion of a board-approved nursing education program; active RN license from the South Dakota Board of Nursing.
Proficiency in Oasis, hospice philosophy, infection control, and various clinical skills such as wound care and IV infusions.
