Lenawee Community Mental Health Authority of Adrian MI is looking to hire a full-time Case Manager (for our Assertive Community Treatment) assigned to work with a specific specialty population, i.e. adults with Severe Mental Illness (SMI), and individuals who have co-occurring disorders meeting criteria for Assertive Community Treatment. A Day in the Life of a Case Manager As a Case Manager , you will assist eligible consumers with the development of an Individualized Plan of Service (IPOS), assist the consumer in the assignment of available agency resources to the plan, and monitor the supports and services according to the plan. In addition, the Case Manager manages agency resources allocated through the Utilization Management System and the Individualized Plan of Service (IPOS), providing the agency with data required for determining consumer satisfaction, treatment and service outcomes and effective, efficient utilization of agency resources. In this role the case manager will:

LCMHA is also a participating / qualifying site for several student debt relief options including the HRSA Loan Repayment Program & MDHHS Behavioral Health Loan Repayment Program (Please note that employee eligibility is determined by these programs, and not by LCMHA).
- Demonstrate a person-centered approach to assessment and treatment.
- Assist a consumer with developing their Individualized Plan of Service (IPOS), and monitor and update their IPOS and perform periodic consumer satisfaction assessments as specified.
- Monitor contract agency treatment plans.
- Maintain positive working relationships with agencies in the community.
- Handle consumer concerns (including intervene, assess, and resolve crisis situations), and convey consumer complaints, grievances and appeals to the appropriate individual(s) according to LCMHA policy.
- Maintain complete, detailed records regarding consumer interactions in the agency's electronic health record within required time frames, consistent with Michigan Department of Health and Human Services (MDHHS) standards and agency procedures.
- Exhibit competency in medication compliance and distribution of medications to consumers.
- Assist consumers with following through on activities of daily living.
- Must attend and complete ACT 101 training within 6 months of hire, and attend a refresher ACT course on an annual basis.
- Carry out additional duties as assigned.
- Bachelor's degree in a human services field from an accredited program;
- Licensed by the State of Michigan to practice in a human services field (if applicable);
- Education, training and/or work experience in the behavioral health field with adults diagnosed with severe and persistent mental illness;
- Basic computer knowledge (such as familiarity with email and internet use) and have experience with Microsoft Office applications (Word, Excel, Power Point, etc.);
- Possess a valid U.S. driver's license, with an acceptable driving record.
- Medical insurance: the employee premium contribution for the plan choices is dependent on the plan and coverage-level choice.
- Vision insurance: the agency covers 100% of the cost for all coverage levels;
- Dental insurance: the agency covers 100% of the cost for all coverage levels.
