Company Description Mysti’s Adult and Family Services WA-provides housing navigation, and community-based wraparound services across the organization partners with Medicaid managed care plans under initiative to support individuals experiencing homelessness, chronic health conditions, justice involvement, and behavioral health challenges. Teams specialize in housing navigation and landlord engagement, community-based case management, benefits enrollment, re‑entry support, and regional center client services. Operating under a Housing First framework, Mysti’s WA focuses on reducing barriers, meeting people where they are, and building sustainable paths to health, housing, and independence. Staff work collaboratively with health plans, county agencies, and community partners to close gaps in care for some of California’s most underserved communities. Role Description This is a full‑time Housing and Employment Case Manager role based in Washington, United States, with a hybrid schedule that combines field work, office time, and some work from home. The Housing and Employment Case Manager will provide direct case management to adults and families experiencing or at risk of homelessness, connecting them with safe housing options, employment opportunities, and supportive services. Day‑to‑day responsibilities include conducting intake and needs assessments, developing individualized housing and employment plans, and assisting with applications for housing programs, benefits, and job placements. The person in this role will coordinate with landlords, employers, community partners, and health or behavioral health providers to support stability and long‑term success. Ongoing duties also include maintaining accurate documentation, tracking outcomes in agency databases, participating in case conferences, and ensuring services align with Housing First principles and relevant program requirements. Qualifications Experience in direct case management, care coordination, or social work with individuals experiencing homelessness, housing instability, or complex health and behavioral health needs. Knowledge of housing resources, landlord engagement practices, local Continuum of Care systems, and public benefits (e.g., Medicaid, SNAP, SSI/SSDI, employment and training programs). Skills in employment readiness support, including resume development, job search assistance, interview preparation, and coordination with workforce or vocational programs. Strong communication, motivational interviewing, and trauma‑informed engagement skills, with the ability to build trust and maintain professional boundaries with diverse populations. Ability to complete thorough assessments, develop person‑centered service plans, document services clearly, and manage a caseload while meeting deadlines and program standards. Comfort working in community‑based settings, including client homes, shelters, and partner agencies, and ability to collaborate effectively in a multidisciplinary team environment.
