Working as a Case Manager you will provide linking, advocacy, coordinating and monitoring to adults with a serious mental illness and/or substance use disorders that are members of our case management team. Our program focuses on helping people improve their mental health by providing person centered care with a focus on recovery and community integration. You will provide culturally responsive services and will work closely with multiple public service systems, including ISK, Kalamazoo Court System, DHHS and the hospitals.

You will have a minimum of one to three years' experience working with adults with serious mental illness, substance abuse and/or co-occurring disorders that also have co-morbid health conditions. You will have a minimum of a Bachelor's degree in Psychology, Social Work, or a related field. A valid Michigan Driver's license is required. Preferred Licensed Bachelors Social Work (LBSW) or Licensed Master's Social Worker (LMSW).
