Physical Therapist Assistant (PTA) - Home Health

Location: Lucas County, OH and surrounding counties

Schedule:

• Monday through Friday
• 8:00 AM - 4:30 PM
• 8-hour shifts, 40 hours per week

Productivity Expectation:

• 30 productivity points per week

On-Call/Weekend Requirements:

• None

Mileage Reimbursement:

• Yes, mileage is reimbursed between patient visits, from the first patient to the last patient of the day.

Pay Rate: $35.00/hourly

Position Overview

We are seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our Home Health team serving patients throughout Lucas County and surrounding areas . In this role, you will provide high-quality in-home therapy services under the direction and supervision of a licensed Physical Therapist, helping patients improve mobility, strength, function, and overall quality of life in the comfort of their homes.

The ideal candidate will have home health experience, excellent communication skills, strong clinical judgment, and the ability to work independently while managing a daily patient schedule.

Key Responsibilities

• Provide skilled physical therapy treatments in patients' homes as directed by the supervising Physical Therapist.
• Implement individualized treatment plans designed to improve patient mobility, strength, balance, endurance, and functional independence.
• Monitor patient progress and accurately document treatment outcomes and responses to therapy.
• Educate patients and caregivers on therapeutic exercises, home exercise programs, mobility techniques, safety recommendations, and fall prevention strategies.
• Observe, report, and communicate changes in patient condition to the supervising Physical Therapist and care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with physicians, nurses, therapists, caregivers, and other healthcare professionals involved in the patient's treatment plan.
• Manage daily travel between patient homes within the assigned coverage area.
• Ensure compliance with all company, state, federal, and payer regulations related to home health services.

Required Qualifications

• Active and unrestricted Ohio Physical Therapist Assistant (PTA) License .
• Previous Home Health experience required.
• Ability to work independently and effectively manage a daily patient schedule.
• Strong clinical assessment, communication, and documentation skills.
• Reliable transportation and ability to travel throughout the assigned service area.
