Essential Duties and Responsibilities: Providescoordination for all patients seeking treatment services at FamilyGuidance Centers, Inc. and screening for all levels of care andassists with the coordination of care. Complete FGC intake screening documents via face-to-face ortelehealth to determine appropriate level of care. Respond to bookings/emails for daily assessment assignments in atimely manner. Administer the biopsychosocial assessments in EHR (SAMMS) withattention to clinical detail and justification from DSM. Complete all intake paperwork including admission note within onebusiness day of the intake. Utilize the SOAP clinical note format for documentation. Coordinate admission process with Retention Specialist to ensuresuccessful completion of all steps by patient (i.e., toxicology,medical team/nursing). Communicate to appropriate staff/referral sources as needed. Schedule: Monday - Friday (7:00a - 3:30p) with one weekend day weekly Start Date: ASAP Pay Rate:$20.19 - $24.04 per hour; based on certificationand experience Family Guidance Centers, Inc. is a Drug Free Environment (includingTHC and/or CBD). Experience and Skills: Education and/or Experience: High School Diploma/GED required; Experience in/with substance use, detoxprograms, medication-assisted treatment, behavioral healthcare and/orcommunity relations encouraged to apply . Certificates, Licenses, Registrations: IAODAPCACertified Alcohol and Other Drugs Counselor (CADC) required,or able to obtain within allottedtime of hire date.
