RN - Case Manager

Case ManagerThe Case Manager will work across various units and will be responsible for managing a caseload of approximately 20 clients. This role requires flexibility and adaptability as it involves floating between different units as needed.Key ResponsibilitiesManage and coordinate care for clients across multiple units.Assess client needs and develop appropriate care plans.Collaborate with multidisciplinary teams to ensure comprehensive client care.Maintain accurate documentation of client progress and outcomes.Conduct regular follow-ups to monitor client needs and adapt care plans accordingly.Schedule RequirementsWeekends are required every other weekend.Block schedules will be implemented as part of the role.On-call duty is not a requirement for this position.QualificationsProven experience in case management or a related field.Strong organizational and communication skills.Ability to manage multiple priorities and adapt to changes.Knowledge of relevant regulations and best practices in case management.Special RequirementsThis position does not permit a work schedule of 48 hours.For candidates interested in making a significant impact on client care through effective case management, we encourage you to apply.
