- Be an integral member of the center and market leadership team, responsible for steering the market towards achieving its quality, utilization, and engagement targets. Participate in leadership meetings alongside the Medical Director and represent our organization to community-based organizations, hospital systems, and payer partners.
- Deliver high-quality primary care to a panel of members, managing chronic conditions, acute exacerbations, and follow-up needs post-hospitalization or emergency care.
- Collaborate with an interdisciplinary team (including behavioral health, nursing, social work, pharmacy, nutrition, and care management) to develop and modify patient care plans.
- Coach and mentor physicians and Advanced Practice Providers on best practices in value-based and evidence-based care.

- Provide on-call coverage on a rotating schedule.
- Allocate approximately 70-80% of your time to direct clinical care.
- Perform additional duties related to chronic care and disease management services as required.
- Possession of an active, unrestricted medical license in the state of employment.
- Minimum of 5 years of clinical experience, including administrative experience leading interdisciplinary teams.
- Experience in a clinic, community-based organization, or inpatient unit.
- Background in training and mentoring others.
- Strong skills in problem-solving, project management, and organization.
- Self-motivated, independent thinker with an interest in population health, healthcare utilization management, and the continuous improvement of our care model.
