PRN Home Health PT (Physical Therapist)Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health PT (Physical Therapist) to work with our Home Health team to deliver individualized care to patients in their homes in Hernando County (including Spring Hill, Weeki Wachee, Brooksville, Ridge Manor, and surrounding areas).Award-Winning Home Health: Recognized as one of America's Best Home Health Providers for 2026, Empath Home Health delivers high-quality, outcomes-driven care trusted by patients, families, and healthcare professionals. Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.Locations: Home health visits throughout Hernando County (including Spring Hill, Weeki Wachee, Brooksville, Ridge Manor, and surrounding areas)Schedule: 5-10+ home health visits per week. Required weekend/holiday on-call rotations.
