Schedule: Monday – Friday | 8:00 AM – 4:00 PM Pay Rate: $15 per hour Case Coordinator – Pyramid Healthcare at Duncansville is committed to providing comprehensive, client‑centered treatment for adults facing Substance Use Disorders. In this role, you will play a vital part in supporting patients’ transitions of care by coordinating aftercare services, maintaining communication with referral sources, and ensuring continuity throughout the treatment process. Key Responsibilities Coordinate aftercare planning to ensure patients have access to appropriate next levels of care. Schedule appointments, meetings, and follow‑up services for patients. Arrange halfway house interviews and placements as part of discharge planning. Communicate with referral sources, families, and external agencies regarding patient progress (as appropriate). Obtain and maintain necessary documentation, releases, and records. Support patients in navigating resources to promote successful recovery outcomes. Maintain accurate records and documentation related to case coordination activities. Collaborate with clinical, medical, and operational teams to support patient care. Perform other duties as assigned. Qualifications High school diploma or equivalent required. Experience in behavioral health, case management, or healthcare coordination preferred. Strong organizational and time‑management skills. Effective communication and interpersonal abilities. Ability to work in a structured, fast‑paced inpatient environment. Basic computer and documentation proficiency. Must pass a criminal background check and drug screening as required by state and federal regulations.
